Description

4.2 Dosage and Method of Administration
Abstral Sublingual 100 mg should only be administered to patients considered to be tolerant to opioid therapy for persistent cancer pain. Patients may be considered tolerant to opioids if they are taking at least 60 mg of oral morphine daily, at least 25 micrograms of transdermal fentanyl per hour, at least 30 mg of oxycodone daily, at least 8 mg of oral hydromorphone daily, or equianalgesic doses of another opioid for more than one week.

Abstral Sublingual Tablets 100 mg should be administered directly under the tongue in the deepest position. Abstral sublingual tablets should not be swallowed and should be allowed to completely dissolve in the sublingual cavity without chewing or sucking. Patients should be instructed not to eat or drink anything until the sublingual tablet has completely dissolved.

For patients with dry mouth, water can be used to moisten the oral mucosa before taking Abstral.

Dose Escalation:

The purpose of dose escalation is to determine the optimal maintenance dose for the ongoing treatment of breakthrough pain episodes. This optimal dose should provide adequate analgesia with an acceptable level of side effects. The optimal Abstral dose is determined for each individual patient by titration. Multiple doses are available for dose escalation. The starting dose of Abstral is 100 micrograms and should be increased as necessary within the range of available dose strengths. Patients should be closely monitored until the optimal dose is achieved.

Switching from other fentanyl-containing products to Abstral should not be done in a 1:1 ratio because of different absorption profiles. A new dose titration with Abstral is necessary if the patient is switching from another fentanyl-containing product.

The following dosing regimen is recommended for titration, but in each case the physician should consider the patient’s clinical needs, age, and concomitant diseases.

All patients should begin treatment with one 100 microgram sublingual tablet. If adequate analgesia is not achieved within 15-30 minutes after administration of a single sublingual tablet, an additional (second) 100 microgram sublingual tablet may be administered. If adequate analgesia is not achieved within 15-30 minutes after the first dose, consider increasing the dose to the next higher tablet strength for the next breakthrough pain episode (see diagram below). Dose titration should continue gradually until adequate pain relief is achieved and side effects are tolerable. The dose strength of the additional (second) sublingual tablet should be increased from 100 to 200 micrograms for doses 400 micrograms or greater. This is shown in the schedule below. During this titration period, do not give more than one dose for any one breakthrough pain episode.
